HOW TO APPLY

All waivers and some credentials, certificates, and permits require you to file the application through your employing agency or institution of higher education. Specific requirements can be found in the Credential Requirements section of this website.

Your Application Packet
First-time applications for credentials, certificates, permits, or waivers must include all of the following:

  • a completed application (form 41-4) [PDF] or waiver request form (waivers can only be submitted by employing agency)
  • any supporting documentation (i.e., transcripts, exam scores, etc), *
  • if submitting Livescan: the second copy of the "Request for Livescan Service" form must be attached to the application packet as proof that fingerprints were taken and the fingerprint processing fees paid
  • if submitting fingerprint cards: submit two completed fingerprint cards (FD-258) and applicable fees. Fingerprint cards may be ordered through the Commission’s automated phone system at 888-921-2682.

Subsequent applications for credentials for credentials, certificates and permits must include all of the following:

  • any supporting documentation (i.e., transcripts, exam scores, etc.) **


Character and Identification Clearance (form 41-CIC)
Individuals must complete an Application for Character and Identification Clearance Form [PDF] (41-CIC) at the same time they submit fingerprint information to the Commission. Submission of the 41-CIC form with a credential application packet will not result in the issuance of a Certificate of Clearance. Instructions for applying for a Certificate of Clearance are explained below.

Certificates of Clearance
A Certificate of Clearance is a document that individuals must obtain prior to beginning student teaching per Education code 44320 (d) and Title 5 Regulations 80028. This document verifies that the individual has completed the fingerprint and character and identification process and has been cleared by the Commission to begin student teaching. Individuals normally apply for the Certificate of Clearance through the college or university in which they are completing their teacher preparation program.
